Why is my sent Smart Form read only?

If you have sent a form to a patient intending it to be used as a Smart Form, but the patient receives it in a non-editable state,  it is most likely that you have opened the form first, and then sent it via Direct Message.

Smart Forms must be sent directly from the favourite forms page by clicking on the DM icon at the end of the row of the form you wish to send:


If you click on the name of the assessment from here to open the form and then select the DM icon at the bottom of the screen, this will send a regular read-only assessment to the patient.  They won't be able to complete it and send it back to you:
